The Wellmore Centre provides Behavioral Health (BH) and Substance Use Disorder (SUD) services under the certification standards of the Ohio Mental Health and Addiction Services department (OhioMHAS).

Clients in care will receive BH and SUD services in accordance with their initial assessment and treatment plan.  These services can occur in the office and the community. All services will be individualized to meet the needs of the client, and should that client object to services, or request a change in service provider or location, the Wellmore Centre will work diligently and collaboratively with all parties, to make any necessary changes to accommodate such requests.  Further, as provided in OhioMHAS regulations, any and all clients are entitled to a second clinical opinion on treatment matters, at their own expense and as arranged by them.  All services and any related accommodations, will be described in the treatment plan.

As outlined in the Consent for Treatment, all clients are expected to participate in treatment planning and the process of treatment. Should at any time a client decides to seek services elsewhere, or it become mutually evident that services from another entity are suited more appropriately to the needs of the client, Wellmore Centre staff will work collaboratively with the client and a new treatment provider, to ensure continuity of care.  Finally, as the Wellmore Centre does not have an ADA accessible bathroom, any client for whom this creates an issue, will be referred to another community provider with such access.

Treatment is voluntary and a plan for treatment is collaboratively developed with the client and the assigned service provider(s). A plan for treatment may be changed and/or modified over time and this is a process that is also mutually agreed upon.

Clients are expected to keep scheduled appointments. In the event that an appointment cannot be kept, we ask clients to contact the office at least 24 hours ahead of time to cancel/reschedule. If attendance problems persist (3 consecutive no-shows), a client’s case may be made inactive and the case may be closed due to a failure to return for treatment. Our office may attempt to contact clients to discuss their situation prior to a case closure for non-participation. Once inactivated, in order to re-engage in treatment, clients are required to meet with a counselor one-on-one; the completion of new intake paperwork may be required; program expectations will be reviewed; clients may be re-assessed and a revised treatment plan will be created with the client.

Clients are expected to not be under the influence of drugs or alcohol at the time of service. Smoking is not permitted in the building and only permitted in designated outside areas. Maintaining confidentiality is a necessary condition for treatment to be effective. Clients who are receiving treatment services in group therapy are expected to keep information and the identity of other group members confidential. Clients are expected to arrive to appointments on time. Clients who arrive late for group therapy appointments may not be permitted to join the group on that day, as this can be disruptive to others receiving therapy. Clients receiving group therapy are asked to remain respectful and supportive of others attending group sessions.

Clients are not permitted to self-administer medications while on Wellmore Centre property.

In the event that the Wellmore Centre must involuntarily discharge a client from treatment (a client who may pose a threat to others, brings drugs into the clinic, etc.), the client has the right to appeal this decision to the executive director.
